Who can sign my CG-719S Small Vessel Sea Service form?

Ship's master, owner, or office staff can sign your CG-719S. You can also sign if you own the vessel.

There are five main positions at your organization who can sign your CG-719S small vessel sea service form. You can sign as the owner of the vessel or it can be the ship’s master, owner, vessel manager or office staff for the company that owns the vessel.

Can I sign my own CG-719S small vessel sea service form?

Yes! You can attest to your own service on vessels you own. You need to sign both sections of the CG-719S small vessel sea service form. Don't forget to complete the bottom part of section III when you signed twice.

What additional documentation is needed for attesting to my own sea service?

If you are documenting sea time on a vessel you own you will always need to provide proof of ownership for every vessel you are submitting to the USCG.

The vessel ownership documentation must list your name and match the vessel identification information you used on your CG-719S. Acceptable proof of ownership may include, although not limited to:

  • Vessel documentation
  • Title
  • Proof of insurance
  • Bill of sale
  • State registration

Who else can sign my CG-719S small vessel sea service form?

The ship’s master, owner, vessel manager or office staff for the company that owns the vessel can sign your CG-719S small vessel sea service form.

You will sign the top part of section III on the CG-719S small vessel sea service form. The person attesting to your service will need to complete the bottom part of section III and sign.

Is any additional documentation needed if someone else attests to my sea service?

No. You do not need to provide any more documentation other than a perfect CG-719S small vessel sea service form if the ship’s master, owner, vessel manager or office staff for the company that owns the vessel signed.
